PARTICIPANT APPLICATION


UP is a post-high school program for adults with special needs. In Texas, there are programs in its school systems through age 21. Therefore, we will accept applications beginning at age 20 for placement after age 21.
TO BECOME AN UP CITIZEN, APPLICANTS MUST COMPLETE THE FOLLOWING 5 STEPS:
Step 1 - TOUR
Take a glance at our innovative vocational program to better understand UP culture and our goals as an organization. Contact Executive Director Sharon Cox at 903-714-3123 to set up a visit.

Step 2 - APPLICATION
We are currently accepting applications for our program. You can download one below. Complete and mail to PO Box 1629, Van, TX 75790.

Step 3 - INTERVIEW
Take part in an interview with the UP Director.

Step 4 - EVALUATION/VOLUNTEER DAY
Upon completion of the UP application and initial interview, a committee will evaluate the applicant and come to an enrollment decision following a volunteer day. The volunteer day will be an opportunity for a prospective Citizen to participate for a day in the program.

Step 5 - ENROLLMENT
If we believe our program is a good fit for the applicant, the applicant will be notified of acceptance into our program.
